How to make an accessible PDF form

Label every field
Each field needs a programmatic label — a tooltip or associated text — so the reader announces "Email address, edit" rather than "edit".
Logical tab order
Set the tab order to follow the visual flow so keyboard users move through the form naturally.
Group related fields
Group radio buttons and checkboxes so their shared question is announced with each option.
